Enrollment Process
Registration
Beginning February 2026
The following items are required at time of registration:
- 2026-2027 Registration Form
- Extended Care Registration Form (if required on regular basis)
- Registration Fee (New Students: $50 & Returning Students: $40)
- One Month Tuition Deposit (Refer to the Tuition Page for your specific program’s rate)
Please Note:
- Your registration fee is due at time of registration and is not refundable.
- A one-month tuition deposit is also due at registration and is refundable if written notice is given by July 1, 2026. Any fees paid after July 1, 2026 are not refundable.
- Registration is not considered complete until all forms and fees have been received.
Enrollment
Summer 2026
Upon admission to Dubuque Montessori School, the following forms must be completed and submitted in accordance with the Department of Health and Human Services Licensing Standards before a child may begin. Our enrollment packet will be mailed to families in early June.

- Parent Emergency Medical Consent Form
- Child History
- Student Physical Form (Completed & Signed by Physician)
- Iowa Department of Public Health Certificate of Immunizations
- Parent/ School Agreement
- Travel, Activity & Media Release Form
- Permission to Apply Sunscreen
- Child and Adult Food Care Program Application and Enrollment Form
- Signed Statement Acknowledging Review of the Dubuque Montessori Parent Handbook
